Oyun, Rapture adında 1960'lardan kalma normal bir sualtı şehrinde geçiyor. Andrew Ryan zamanında bu sualtı şehrini sanatçıların, bilim insanlarının ve düşünürlerin üzerlerinde hiç bir baskı hissetmeden çalışabilmesi için kurmuş, ancak daha sonra bazı şeylerin ters gitmesi sonucu Andrew Ryan'ın bu projesi tam bir kabusa dönüşmüş...

(Hikaye başlığında daha fazla ayrıntı bulabilirsiniz.) Karşımıza önemli iki şey çıkıyor, ADAM ve para. Parayı çevreden veya öldürdüğünüz kişilerin üzerinden bulabiliyorsunuz ve parayı Circus Of Values vb. makinalarda sağlık kiti, çikolata, cephane vb. satın almak için kullanıyoruz. ADAM ise Plasmid adında özel güçler için gerekli, ve bu ADAM'ı sadece Little Sister adında küçük bir kız üretiyor. ADAM bulmak gözünüze oldukça kolay gözükmüş olabilir. Sonuçta küçük bir kızdan şeker çalmak gibi. Ancak Little Sister'ların yanında Big Daddy adında dev dalgıç kıyafeti içinde ağır ağır dolaşan karakterler bulunuyor. Ve Little Sister'dan ADAM'ı alabilmek için bu karakterleri öldürmelisiniz. Bu da hiç kolay bir şey değil. Big Daddy'lerin zırhları oldukça kalın ve onları öldürmek için gelişmiş silahlar kullanmalısınız. Big Daddy öldükten sonra, Little Sister artık elinizde oluyor. İster Harvest seçeneğiyle paraziti bütün haliyle ele geçiriyor ve 160 ADAM kazanıyorsunuz. Diğer seçenek olan Rescue yi seçerseniz, paraziti öldürüyor ve kızı kurtarıyorsunuz fakat 80 ADAM kazanıyorsunuz. Ancak burada yaptığınız seçimler oyunun sonunu etkiliyor.
